Full Job Description:
AURORA Home Health is seeking an experienced Home Health Registered Nurse for field visits in CENTRAL DALLAS, TX area. Ideal candidate will have a year or more of recent home health experience. Must have clinical assessment skills and knowledge of OASIS.

About our Company:
AURORA Home Health is an independent Home Health Agency licensed by the State of Texas and Medicare certified. AURORA Home Health agency offering Medicare-Certified Home Health care services throughout the DFW.

About the Position:
— Registered Nurse are responsible for comprehensive assessments in the patient's home, including the completion of OASIS using electronic documentation (Axxess).
— Evaluate patient and home health environment to determine required services and involve the patient and patient's family in developing the plan of care.
— Develop the POC based on findings from evaluation visit.
— Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care.
— Administer medications and treatments as prescribed by physician — Provide direct supervision of nursing clinical care team members (LVN's & HHA's).
— Participate in regular case conference and in-services

Requirements:
— One year of previous adult home health experience required
— Current Texas Registered Nurse (RN) license
— One year of clinical nursing experience
— Current Texas driver's license
— CPR Required
— Experience with healthcare software, Axxess is a plus
— One year of previous OASIS experience
